Dear KMPTPALS

I refer to the Care Plan incorporating the diagnosis from Dr Oranusi at Britton House as attached:  FrBrittonHouse(CarePlanfromEmmanuelOranusi) 29April 2024.docx.

This early evening I have had a moment of reflection as linked here: https://www.thelibertariandemocrats.com/forum/diary-of-the-author/18-may-2024-18-58-pm-uk-time after I had published the book ‘So what now’ in its short but final form, and blocked Jill Jesson the psychotherapist clearing all the Chat conversation that took place with her so that there is no going back to pretending or accepting that I am mentally ill with a split mind. All my interactions and conduct were always tactical methods to withstand State-organised persecution on me and this has been the case since 1998 when I was in the University of Greenwich that was instituting gross misconduct disciplinary charges against me and I had to avoid the kangaroo court to protect my reputation. I will fight any insinuation that I am. I will take the old medication regime before the 22 April appointment with Dr Oranusi and ignore the Care latest Care Plan from Dr Oranusi so will not attend any further appointments at the Community Mental Health Clinic. I repeat that I am not paranoid schizophrenic or depressed or have mood instability issues. I am very sane indeed. I only take the mental health medications because my wife Rashmi asks me to take them. That is all.

If you have any questions, please write to me by email because telephone or face-to-face talk is cheap rubbish that hide the ill-intentions of the person interrogating one. This I have learnt from bitter experience.

Yours sincerely

Dr Shantanu Panigrahi

Review:

It was nice seeing Mr Panigrahi in company of his wife in my clinic at Britton House on the 22nd of April 2024. He reported that he is almost always anxious and agitated. He continues to be paranoid, believes people are out to harm him. His wife tells me that he misinterprets things and believes much in conspiracy theories. He is worried that we are planning to discharge him back to GP. He was reassured when I told him that we have no plan to do that at the moment. His wife hinted that she had to come with him, otherwise he would not have come on his own as he believed that something bad would happen to him. He believes that people are terrorizing him. He informed me that his case with the police is settled and that he has no case to answer. He is compliant with meds and appears to be be willing to continue taking treatment from us. He no longer believes that his schizophrenia has been cured. He has had about 7 sessions of psychotherapy privately and not sure whether it was helpful or not.

He lives with his wife and daughter and is unemployed. He smokes about 15 cigarettes a day, says he does not enjoy it, but indulges in it due to boredom. He denies any use of illicit drugs or alcohol.

On mental health examination, he appeared casually dressed and tidy. He was very restless especially in his legs, but did engage well with me. His speech was fairly normal and coherent and he did explain his current symptoms reasonably alright. He definitely was a lot more co-operative and amenable and did not engage in arguments as was the case when I spoke to him on phone previously. He was not acutely unwell and no expression of thoughts of self-harm. He wants to get well and is willing to continue taking treatment. He did not appear to be vulnerable but could be when acutely unwell due to paranoia or being in delusional state. His insight around the illness has improved and he does have capacity to give informed consent on his treatment. I have given him a script for procyclidine 5 mg to be taken thrice a day and aripiprazole 5 mg daily, advised to continue with other meds as prescribed, and GP to please continue to give repeat scripts. He will be seen again in 3 months.

Yours sincerely
